REQUEST INSURANCE AUTHORIZATION

We accept BCBS, Tricare, Medicaid, Step Up for Students, and private pay for ABA services. We accept Tricare, Step Up for Students, and private pay for Speech & Language Services. We need the following information emailed to us to begin the process:
- ABA Services:
  
  Referral from your child’s doctor with the following:
  
  Child’s Name
  
  Diagnosis Code F84.0 with severity level 1, 2 or 3
  
  Statement indicating need for ABA Therapy
  
  Doctor’s signature and NPI
  
  Diagnosis paperwork stating assessments completed, scores and results
- Speech and/or Language Services
  
  Referral from you child’s doctor with the following:
  
  Child’s Name
  
  Statement indicating need for Speech and/or Language Therapy
  
  Doctor’s signature and NPI
SCHEDULE AND COMPLETE ASSESSMENT APPOINTMENT
- An appointment for us to come to your home to complete an assessment with you and your child needs to be scheduled. This appointment takes approximately 1-2 hours.
- Assessment(s)/Questionnaire(s) will be emailed for you to complete to help with developing a plan for your child.
PARTICIPATE IN DEVELOPING A PLAN
- A professional will compile all assessments and data and write an individualized plan to address your child’s needs.
- Plan will be reviewed with you to ensure agreement
- You will sign the plan and it will be sent to insurance for approval
WORK ON GOALS
- A professional will come to approved environments on the agreed upon days and times to work with your child and family on goals (once the plan is approved by insurance)
- Parents and/or guardians will participate in implementing the goals from the plan in all approved environments.
- Parents and/or guardians will attend parent trainings at least once a month too ensure understanding of the implementation of the plan.
MASTERY

Our goal is to work ourselves out of a job. We know you don’t want us in your life forever. The plan will be adapted to best meet the needs of your child and your family for as long as needed. Once their plan has been mastered and your child no longer needs our services they will graduate from Therapy Junction!
